How to Refer to Partnership for Good Health

Partnership for Good Health (PGH) accepts referrals for Cuyahoga County residents of any age or gender who have a language barrier and need support navigating the healthcare system.

Who should be referred?

A referral may be appropriate when the client:

• Lives in Cuyahoga County

• Has a language barrier and does not speak English

• Speaks one of the languages represented by the PGH Community Health Worker (CHW) team:
  Arabic • Dari • Kibembe • Nepali • Pashto • Swahili

• Needs help accessing, understanding, or navigating the healthcare system

Before making a referral

Please talk with the client before submitting a referral. The client should:

• Know that a referral is being made and give permission to be referred

• Understand the role of a Community Health Worker- these short videos can also help explain and are available in multiple languages:  Introduction to PGH and What is a Community Health Worker

PGH Community Health Workers share language and cultural understanding with the communities they serve. They help clients navigate the healthcare system, connect with services, understand next steps, and address barriers that may make it difficult to access care.

How to refer

Referrals are submitted through the Better Health Partnership Pathways Community HUB.

The HUB reviews referrals and, when appropriate, assigns clients to Partnership for Good Health based on their needs and preferred language.
