Referrals

PGH accepts referrals from healthcare, social service, and community organizations.  We accept clients who have a language barrier to healthcare as long as we have CHW who speaks their language.  We ask that before referring someone to PGH, please make sure they are interested in receiving assistance from a CHW who will speak their preferred language.

How to make a referral?

PGH is now a coordinating agency for the Better Health Pathways HUB.  Referrals can be through this online Referral Portal.

Once we receive a referral, our CHWs will contact the potential client to explain and offer our services.  If they agree, a home visit will be scheduled to complete the enrollment process.

Enrollment may take 3-4 weeks depending on the availability of the CHW and client.
